  2. )  Attach a copy of your unexpired driver's license (both if a joint application).*
  3. )  Enclose a check or money order made payable to "The State Credit Union" in the amount of $7.00.

Please submit your completed application and all supporting documentation to the address indicated on the application or you can fax it to 304-558-0137. If you have any questions or require assistance with completing your application, please contact the Credit Union at 304-558-0566

*If the address listed on your driver's license does not match your physical address, please provide (2) two examples of your current physical address (example would be a copy of a utility bill).
